Leslie R. Collins, 77, of Westerville, OH passed away Friday, Feb. 1, 2013 at Mt. Carmel-St. Annes in Westerville, OH.
He was born Nov. 30, 1935 in Coolville, OH son of the late Kelly and Bertha Ward Collins.
He is survived by a sister, Jean Welsh of Coolville; a niece, Rita Seals and several other nieces and nephews.
In addition to his parents, he was preceded in death by a half brother, Mayford Cook.
Services will be held at 2 P.M., Tuesday, Feb. 5, 2013 at White-Schwarzel Funeral Home, Coolville, OH with Pastor Helen Kline officiating. Burial will be in the Coolville Cemetery.
Friends may call at the funeral home Tuesday one hour prior to the service.
